Step 1: Delete the Login from the Old Record
- Navigate to the Patient Details tab in the old record from the office the patient was discharged from.
- Once you are in the Patient Details tab of the old record, look for the vertical ellipsis icon (three dots icon).
- From the dropdown menu, click on Delete Login. This step will ensure that the patient's previous login details are removed and will help in creating a fresh account for the patient at the new office.
Step 2: The patient creates a new account in the EMBODI app

The patient needs to create a username and password in the EMBODI app as if registering as a new user. They can use the same username and password as before.

The patient would need to click Sign Up instead of Login

Once the patient successfully created their EMBODI app account, the provider can verify that they are in their system
Step 3: Verify Appointment Access
- After the new account has been created and linked to the new record, verify that the patient can now see their current and future appointments.
- You can either test this by accessing the patient’s account directly or by guiding the patient to check their own access through the app.
By following these simple troubleshooting steps, you can ensure that a patient who has been transferred from one chiropractic office to another will regain access to their current and future appointments. It’s crucial to ensure the migration of their account to avoid any confusion or delays in scheduling.
